Active Tennessee Physical Therapy license is required to start the assignment. Current CPR certification. Valid driver’s license, current vehicle insurance, and access to a dependable vehicle for patient visits across the agency service area. Experience providing home health physical therapy services or equivalent outpatient/community-based care preferred. Ability to complete patient evaluations, develop plans of care per state guidelines, and submit documentation within required timeframes. Willingness to participate in weekend rotation and be available on observed holidays as needed. Ability to supervise PTA and home health aides when permitted by state regulation and agency policy preferred. Assignment Details Facility type: Home Health. Shift name: Other.
