Job Description:
A company is looking for a Nurse Practitioner to perform in-home
health assessments on Medicare Advantage Members in Kentucky.
Key Responsibilities
Conduct in-home health assessments for Medicare Advantage members
in designated counties
Complete annual health assessments ranging from 45-90 minutes in
duration
Work flexible hours, including weekends, based on personal
availability
Required Qualifications
Minimum of 6 months experience as a Nurse Practitioner
Active and unencumbered Nurse Practitioner license and national
certification
All board certifications accepted except PMHNP or WMNP
Experience in gerontology, cardiology, internal medicine, or
endocrinology is preferred
Home health care or home visit experience is a plus
